Vision

• Coast to Coast provides 50,000 optical providers nationwide, eye exams, retail and mail order contacts and LASIK surgery in certain areas.
• AmeriSight, a vision correction management company, has a nationwide network of Board Certified Ophthalmologists to perform Basic and Custom LASIK surgery. Savings up to 55%.

Prescription

• Medco Health provides retail and mail order access to prescription drugs. Their services include a special prescription monitoring service call drug utilization review which will alert a pharmacist to important information that may affect the dispensing of a prescription.
• AmeriPlan® Prescription Advocacy Program – Administered by EZMeds. To qualify for this advocacy program, monthly medications must cost over $90 per month and member must meet the income requirements. If a member qualifies for the program, they are charged a nominal monthly fee to receive all their maintenance prescriptions at no cost.

Chiropractic Services

• Proprietary Network – All chiropractic services have reduced fees with no charge for first consultation. Many chiropractors have instituted Wellness Programs into their practice.

Hospital Confinement/Patient Advocacy Program
Administered by The Karis Group. Members with hospital related costs of $2500 or more have access to a Patient Advocate who will negotiate with the facility and healthcare providers on their behalf.

Mental Health

• Let’sTalkCounseling.com is an online mental health program with a nationwide database of professional licensed counselors, therapists and online counseling providers.
• Proprietary network has several rehab and mental facilities as well as individually contracted mental health providers.

Nurseline – Registered Nurses give AmeriPlan Health® members immediate access to healthcare professionals through a toll free number. Unlimited 24/7 access at no additional charge to members.

Wellness Program

• Infuse Health administers the AmeriPlan Health® Online Wellness Program. A highly qualified panel of physicians, exercise physiologists, nutritionists and dieticians designed these unique programs that will provide guidelines and tracking of progress to help member achieve a healthier more balanced life.

• Diabetic Supplies – Administered by Diabetic Solutions, Inc. Accessed through a mail order program with free shipping, AmeriPlan Health® members can save hundreds of dollars per year. Members can choose the program that fits their medical requirements. To utilize, call toll free number.
• Diagnostic Imaging & Radiology – AmeriPlan Health® has contracted with several large diagnostic imaging networks and individually owned facilities. Savings range from 20%-69%. To utilize these facilities, call AmeriPlan® medical customer service.
• Physical Therapy – AmeriPlan® has contracted with Physical Therapy Provider Network (PTPN) with over 1,000 locations nationwide as well as local physical therapy facilities throughout the US. To utilize, call AmeriPlan® medical customer service.

Includes all the programs and providers offered to Consumer Choice #1 and #2 members.

Total Health Plus – Consumer Choice #4

Includes all health care programs (Consumer Choice #3) plus three (3) non-medical discount programs.

Non-Medical Discount Programs

• Auto Club – Administered by United States Auto Club-Motoring Division. Emergency roadside service is provided 24/7. If a member requires towing to nearest destination of Member’s choice or winching (30 minutes), the service is free (Sign & Drive) up to $50.00. Other roadside services available such as flat tire service, out-of-gas services and lock-out services. Personalized trip routing and information calls are also provided services. Members will receive a 25% discount on all charges exceeding $50.00. To utilize, member will be given toll-free number.
• Identity Theft Assistance – Administered by CLC. This program provides members with education and assistance in preventing Identity Theft and resolution after the fact. Personal Advocate assigned to theft victim who will assist in all steps of after the fact resolution. Free consultation with Financial Coach regarding strategies to rehabilitate credit score damaged by identity theft. If legal assistance is required, members will receive a free consultation and 25% discount on attorney fees. One full year of active follow-up to judge effectiveness of the resolution. • Legal and Financial Services – Administered by CLC. Members will have access to the following:

1.       Attorney Services – One initial 60 minute office or telephone consultation at no cost with a network attorney. If attorney retained, member will receive a 25% reduction from attorney’s normal hourly rate. All types of legal matters eligible.

2.       Mediation Services – One initial 60 minute office or telephone consultation at no cost with a network mediator. If mediator retained, member will receive a 25% reduction from mediator’s normal hourly rate. Typical matters include disputes involving divorce and child custody, contracts, real estate/landlord-tenant, car accidents – insurance, etc.

3.       CLC Premium Web Site – Members will have unlimited access to web site services such as legal and financial forms, financial calculators, professionally written articles and a Frequently Asked Questions section.
